Each one is a person choosing an agency. We reach them.

Nobody refers an agency they've never heard of.

You already know how to take care of patients. Your nurses and aides show up, and your intake team knows how to start care fast.

What runs short is introductions. A liaison can only walk into so many offices a week. The case managers, social workers, and practice managers who decide who gets called are on LinkedIn. We reach them there, from your profile, so your name is familiar before anyone walks in.

Who we reach

Titles we target

Discharge Planner · RN Case Manager · Care Transitions Coordinator · Transition of Care Nurse

Why they matter

They build the list of agencies a patient sees before leaving the hospital. When the list is long and the discharge is today, they lean on the agencies they know will pick up the phone.

Reaching discharge planners

Filtered by your service area, the organizations that discharge or refer, and the title that makes the call

Before the first message

Everyone on the list sees your name a few times before you ever write to them. We start with about 20 new people a day and work up to 40 as your account gets used to the volume. The full breakdown is on how it works.

  1. Day 1

    Profile view

We visit their profile from yours. Your name and agency show up in their notifications.

  2. Day 2

    Post like

If they've posted recently, we like it. Now they've seen your name twice.

  3. Day 3

    Second view

One more visit, so your face is familiar when the request comes in.

  4. Days 3 to 5

    Request & message

The connection request goes out. When they accept, they get a first message written from their profile.

  5. 3 days later

    Follow-up

One short, friendly note if they haven't replied. Then we stop.

What the first message looks like

Each one is written from the person's own profile: their role, their unit, the patients they see. Then one line on who you are and one direct question. No links, no brochures. When they reply, you take the conversation from there.

Our job

Start the conversation.

  • Build the list by organization, partner type, and title, inside your service area.
  • Write the first message and follow-up for each partner type, from each person's profile.
  • Run the sequence on your profile, every day, within LinkedIn's limits.
  • Flag every reply to you, and meet every two weeks to go over who replied and who to visit.

Your job

Build the relationship.

  • Have a LinkedIn profile that clearly says who you are and what your agency does.
  • Answer replies the same day, in your own words.
  • Move the conversation to a call or a visit, and make it easy to send you a referral.
  • Show up to the check-in every two weeks.

How it runs day to day

  • Who sends the messages?

We do, from your LinkedIn account, through an outreach platform you connect once. Volume starts low and stays inside LinkedIn's limits.

  • Who approves them?

You. Before launch you see the list and the messages for each partner type. Nothing goes out until you sign off.

  • Who answers replies?

You do, in your own words. We flag every reply the same day with a note on who the person is.

  • What if I stop?

Disconnect the platform and it's done. Every connection and conversation stays on your profile.

Honest timing: months, not days

Referral relationships take repeated contact. Every week, patients in your area leave the hospital, finish rehab, or tell their doctor they're struggling at home, and someone gets that call. Outreach makes sure the people making it know your name.

  1. Weeks 1 to 2

Setup. We tune your profile, map the referral organizations in your service area, and write the messages for each partner type. You approve them.

  2. Month 1

Outreach starts slow and builds from about 20 to 40 new people a day. First connections and replies come in.

  3. Months 2 to 3

Conversations. You're taking calls, answering questions about your coverage, and setting up visits and in-services.

  4. Month 3 on

Partners start testing you with a first patient. How that case goes decides what comes next.
